Thank you for uploading the form template.

Replicated your form template to "Survey Short2", published and created a report called Survey - Short2 without issues.

Could you try to replicate the issue yourself by publishing the form template and create a report through Your Test Area --> Report Manager 1 (or 2).

If you are able to generate the issue, could you let me know which field you have selected when creating the report fields?

If you are not able to create the report field, go back to your system, create a new report with just 1 non-meta data fields selected and see if the issue still exists. In addiiton could you also try with a different form template.

I suspect this may be just a one off issue but needs your confirmation.

Simply creating another (identical) report solved the issue. Thanks.
